Important Notice for Employees: Your 1095-C Forms are Available

For those enrolled in or eligible for medical insurance through the company in 2025.

We are pleased to inform you that your 1095-C forms, which provide medical insurance coverage information for you and any covered dependents, are now available. 

Where to Find Your 1095-C

You can view and download your 1095-C form directly from Workday. 

  1. Log into Workday
  2. Click Personal in the upper left corner of the home page
  3. Select the Benefits and Pay option
  4. Click on benefits, then click ACA Forms

If you prefer to receive a physical copy of your 1095-C form, you can submit a request to the Benefits team by email at benefits@discounttire.com.

NOTE: Be sure to include your name and employee ID in the physical copy request.
